Explain the nature, goals, and expectations of neuro rehabilitation clearly to patients and caregivers. Assist families in accepting long-term rehabilitation needs and guide them through the psychological adjustment process. Counsel family members & attendants on managing stress and offering ongoing emotional support as required. 2. Patient Coordination and Care Navigation: Serve as the single point of contact for patients throughout their rehabilitation journey. Coordinate appointments across departments (e.g., physiotherapy, occupational therapy, speech therapy, neuropsychology). Help families with logistics such as admission process, stay arrangements, discharge planning, and follow-up schedules. Maintain ongoing communication with the interdisciplinary team to track progress and adjust care plans accordingly. 3. Handling Enquiries and Conversion to Admissions: Respond promptly and empathetically to all patient or caregiver enquiries (in person, over phone). Clearly explain available rehabilitation services, infrastructure, packages, success stories, and expected outcomes. Build trust and rapport with patients and family members, addressing doubts, fears, and misconceptions. Conduct facility visits (physical) to showcase the centre’s capabilities and comfort level. Follow up on enquiries through various sources and convert leads into confirmed admissions. Liaise with the finance/billing department to explain cost estimates and facilitate financial counselling if required. Collaborate with marketing and outreach teams to understand and track lead sources and referral patterns. 4. Documentation and Reporting: Maintain accurate records of enquiries, counselling sessions, admissions, and therapy progress. Prepare reports on enquiry-to-admission conversion rates and patient satisfaction. Document counselling session assessments and contribute to case records for holistic care planning. 5. Quality and Service Improvement: Collect and analyse patient and family attendants feedback to enhance service delivery. Identify barriers to admission or retention (e.g., financial constraints, lack of awareness) and escalate to the leadership team. Recommend strategies to improve patient onboarding and rehabilitation experience. 6. Outreach and Community Engagement: Strategize medical camps, hospital outreach events, and awareness drives to promote the Neuro Rehab Centre. Follow up with discharged patients to ensure continuity of care and track long-term outcomes.
