Medical Coder

Key Responsibilities:- Review and analyze clinical documentation from home health and hospice services to assign accurate diagnosis codes (ICD-10-CM). Ensure coding compliance with federal regulations, CMS guidelines, and payer-specific requirements. Validate codes against OASIS documentation for Home Health and Plan of Care for Hospice. Collaborate with clinical staff and billing teams to clarify documentation and support appropriate code assignment. Maintain knowledge of current coding standards, home health/hospice regulations, and changes in payer guidelines. Assist in quality assurance audits and support education initiatives related to coding accuracy. Identify trends or discrepancies that may affect revenue cycle and collaborate with leadership to resolve issues. Required Qualifications:-  High school diploma or equivalent (Associate’s or Bachelor’s degree preferred). Active certification in medical coding such as CCS, CPC, CRC, or COC. HCS-D or HCS-H certification strongly preferred (or willingness to obtain). Minimum of 1 to 2 years of coding experience in Home Health and/or Hospice. Proficient in ICD-10-CM coding and OASIS documentation for Home Health. Strong knowledge of CMS regulations, PDGM (Patient-Driven Groupings Model), and hospice coding practices. Excellent attention to detail and strong analytical skills. Ability to work independently and meet deadlines in a remote/home-based environment. Preferred Qualifications: Experience with electronic health record (EHR) systems such as Homecare Homebase, MatrixCare, or WellSky. Familiarity with HIPAA and data security requirements in remote environments. Strong communication and organizational skills.
