Role Description:

We are seeking a skilled and detail-oriented Medical Coder with expertise in Hierarchical Condition Categories (HCC), Evaluation & Management (E/M), and Minor Surgery coding. The ideal candidate will ensure accurate coding for diagnoses, procedures, and services to support proper reimbursement and compliance with payer guidelines.

Key Responsibilities:

Assign accurate ICD-10-CM, CPT, and HCPCS codes for HCC, E/M, and minor surgical procedures.

Review clinical documentation to ensure completeness and coding accuracy.

Identify coding errors and denials; work with physicians and billing teams for resolution.

Ensure compliance with CMS, AMA, and payer-specific coding guidelines.

Maintain up-to-date knowledge of coding changes and regulations.

Support audits, education, and quality assurance initiatives.

Requirements:

Certification: CPC, CCS, or CIC (AAPC/AHIMA preferred).

1–3 years of experience in HCC, E/M, or surgery coding.

Strong knowledge of ICD-10-CM, CPT, and HCPCS Level II.

Familiarity with EMR/EHR systems and coding software.

Excellent analytical and communication skills.

Preferred Skills:

Knowledge of risk adjustment models (RAF/HCC).

Experience handling denials and rework.

Strong attention to detail and documentation improvement awareness.
