The Responsibilities of the Resident Doctor include the following: 1. The Resident will co-ordinate the activities of other staff in the ward (Staff Nurses / Ward Secretaries / Ward boys, etc) and ensure smooth functioning of the ward activities as a team work. 2. The Resident will conduct a thorough and detailed work-up of the patients at the time of admission and record all findings in the initial assessment case sheet. This documentation should be completed within the first hour of the patient’s admission to the ward. 3. Care plan should be documented in consultation with the primary physician and patient should be educated about his/her condition. 4. The Resident shall make detailed rounds of all patients under his / her care well in advance of the Consultant’s rounds and appraise the Consultant of all findings, progress, treatment plan etc. 5. During the rounds, the current status of the patient should be noted on the progress Sheet and the treatment chart should be reviewed after discussing with the concerned Registrar / Consultant. Clear instructions should be given to the Nursing Staff regarding investigations and plan of treatment. 6. All investigation reports must be entered in the master investigation chart as and when they are received and the reports should be communicated to the Consultant / Registrar. 7. After the visit of the Consultant, his / her findings and instructions are to be entered in the Case Sheet and Nurse Incharge should be briefed accordingly and appropriate action taken regarding Consultant’s order. 8. All Residents before leaving the Hospital should hand over all the patients to the next Resident on duty and brief on investigation required, change of line of treatment and any special instructions. 9. It is mandatory that the Resident on night duty attends to each call personally and documents the status of the patient and the treatment instituted. 10. The Resident on night duty shall inform the Nursing Station / Casualty / Reception of his / her whereabouts in the Hospital. 11. It is unethical to prescribe any drug or treatment on phone without examining the patient. This should be avoided. 12. All Gentlemen doctors should respect the privacy of female patient and ensure that he is accompanied and assisted by a Nurse / Female Attendant / or any other appropriate female person during the examination of a female patient. 13. Patients in ICU‘s should not be left unattended at any given time. 14. Concerned Resident should help the Registrar / Consultant on duty in ICU whenever required in managing critically ill patients. 15. It is the responsibility of the Resident to be conversant with the status of all the patients admitted under the care of the respective Consultants including the patients admitted or transferred to the ICU’s. 16. All discharge summaries pertaining to patients under a Consultant shall be written by the Resident concerned and shown to the Consultant for his / her approval. Discharge Summaries need to be signed by Consultant before handing over to the patient. 17. Patients should not be permitted to go for any interventional procedure / surgery without initial history, physical examination and laboratory work. Non-compliance in this regard shall be viewed seriously. Consent form to be explained and signature taken before the procedures. 18. Residents working in surgical departments must ensure that the pre-operative work up of the case is complete and consent form signed. 19. Resident shall refrain from criticizing their colleagues or Consultants and the plan of treatment instituted in the presence of patients or the patient relatives. 20. Residents should not discuss a patient’s illness with another patient / other people. 21. Any disagreement with the instituted line of treatment, etc. should be discussed with the Consultant concerned. The same holds good about other work situation grievances. Loose talk is to be desisted. 22. The specified doctor will perform any other work inside the premises of the Hospital or outside, in line with Medical Work, as may be assigned to him/her, from time to time by his/her senior colleagues / Management. 23. Resident doctors will be posted for health checkup camps outside the hospital premises as per requirement. 24. The Hospital Policy should be followed strictly and shall not be criticized in the presence of patients and their relatives. 25. In case of an emergency situation, any Resident can be called upon to help in the management of critically ill patients. In such instances no Resident, shall refuse to attend to the patient on the grounds the he / She is not working in the concerned department / section / ward. 26. All Residents must attend the clinical / academic programmes arranged from time to time. Any absenteeism will be viewed seriously. 27. Working for another Hospital / Medical / Health Care Institution while being a full time paid employee of the Organisation is not acceptable or allowed. 28. The Casualty Medical Officer who is summoned to court of law to dispose as an expert witness for Medico-Legal Cases should reschedule his / her duty with his / her colleagues. This applies to all Residents in similar circumstances. 29. The Resident should be conversant with Code Blue Protocol. Residents responding to Code Blue should inform and seek the help of Anesthetists for resuscitation of patients. If in doubt the help of the Registrar / Consultant should be sought without delay. 30. Accumulation of compensatory off is not permitted and clubbing as a result is not permitted. The compensatory off, if not taken the day following night duty, will lapse. 31. All Residents are to sign the Attendance Register kept at an appropriate place before they go to their respective wards / work areas. Habitual late coming and absenteeism is discouraged and will be viewed seriously. 32. The allotted Duty Room shall not be misused in a way to attract derisive comments from passersby, patients and their relatives. 33. The Residents should confirm to the dress code prescribed by the Hospital Management. Jeans and ‘T’ Shirts are not to be worn inside the Hospital. 34. The Resident should wear Doctor’s white coat while functioning in the patient care area. 35. The Duty Doctor will perform any other duty / task assigned to him/her by his/her superiors. Responsibilities of the Resident Doctor at United Hospital: 36. Initial Assessment/ Documentation: As soon as the patient gets admitted, the doctor should meet the patient, introduce themselves and take relevant history and document the same. 37. Documentation and its timing is a key performance indicator (KPI) for NABH. 38. If the doctor is occupied with some other patient or bedside work, he or she should promptly meet the patient and introduce themselves and inform, that they will be back in some time for the assessment. 39. A friendly, approachable professional attitude with the patient and the attenders is a must. 40. All handovers should be taken bedside with case files. 41. Handover register should be updated during every shift change. 42. Inform the concerned admitting Consultant, (In-house/ Visiting Consultant) about the admission as soon as you receive information and take instructions, whether mentioned or not. 43. During communication, clearly mention complains, past history and about the medications which the patient is already receiving before admission. 44. Check if all the investigations advised at the time of admission are done and if not arrange for the same. If any report is found abnormal, inform the respective consultant or the Anesthetist on call. 45. It is the responsibility of the resident doctor to follow up and collect the report at the earliest and inform the consultant. 46. Check and follow up on the status of PAC and Physician fitness if advised. 47. The consultants should be Updated about the progress of the patient during hospital stay once in the morning 10 am to 11am and once in the evening 7pm to 8pm. 48. The discharge summaries should be typed properly and cross checked especially the operative notes and the advice on discharge. It has to be verified by the Consultant. 49. The advice on discharge should be clearly explained to the patients prior to discharge. 50. Most of the Surgical cases will have a DJ Stent/ Implant. The discharge summary should mention about this clearly and instructions regarding its presence, care, removal plan, precautions should be explained. 51. If the discharge process and instructions from consultant are planned during rounds it should be prepared by the same doctor or clear instructions to be documented in handover book and to be handed over to Doctor in the next shift. 52. Resident Doctor has to enquire with consultant and instruct patient and attenders regarding Follow-up Consultation date. 53. Resident doctor’s role is very important in the outcome of the patient. We expect them to be highly responsible.
