Role Description

This is a full-time, on-site role for a Care Manager located in Hyderabad. The Care Manager will oversee and manage care plans tailored to clients' health and wellness needs. Responsibilities include coordinating with caregivers and healthcare professionals, assessing patient requirements, monitoring progress, and ensuring the delivery of high-quality, personalized care. The Care Manager will also maintain clear communication with clients and their families to ensure satisfaction and adherence to care plans.

Qualifications

• Experience in care coordination, patient assessment, and case management
• Strong knowledge of elder care, home healthcare, and rehabilitation services
• Excellent communication, interpersonal, and problem-solving skills
• Leadership and organizational skills to effectively manage care teams
• An empathetic, patient-centric approach and the ability to build trust with clients and families
• Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field; additional certifications in healthcare or care management are a plus
• Proficiency in documentation and knowledge of basic health and safety protocols
• Prior experience in a similar healthcare role is preferred
