Care Manager (Full-Time | On-Site |Mysore)

With operations across the country and a strong presence in Hyderabad, we are now expanding our footprint in Mysore. We are looking for experienced, self-driven Care Managers who are passionate about improving the lives of elderly clients through organized and empathetic care delivery.

About the Role

As a Care Manager, you will play a key role in ensuring quality care for our senior clients by coordinating between families and our team of home healthcare professionals. You will be responsible for client onboarding, supervision of care delivery, home visits, and managing relationships with both clients and caregivers.

Key Responsibilities

• Placement and Deployment: Assign and manage home health workers as per client needs.
• Feedback and Service Improvement: Gather and assess feedback from clients and families to enhance care delivery.
• Care Plan Development: Collaborate with clinicians to prepare and update individualized care plans.
• Assessment and Monitoring: Conduct home visits, assessments, and periodic client check-ins.
• Relationship Management: Build strong relationships with clients and caregivers to ensure satisfaction and continuity of care.
• Team Support: Maintain open communication and support for field care workers to uphold service quality.
• Payment Collection: Ensure timely and accurate collection of payments as per company policy.

Candidate Requirements

• Educational Qualification: Physiotherapist/ GNM / B.Sc. Nursing/ paramedical qualifications
• Experience: Minimum 3 years in a healthcare or home care setting; candidates with team management experience preferred
• Mobility: Willingness to conduct home visits; owning a two-wheeler is preferred
• Skills: Strong interpersonal and organizational skills; ability to work independently and in teams

Job Types: Full-time, Permanent

Pay: ₹25,000.00 - ₹35,000.00 per month
