Key Responsibilities Conduct regular visits to members' homes to assess their wellbeing and requirements. Coordinate healthcare and support services as per the members care plan. Liaise with family members, doctors, hospitals, and service partners. Maintain detailed care records and update internal systems regularly. Handle member queries and provide empathetic, solution-oriented support. Assist in onboarding new members and conducting care assessments. Requirements Graduate in any discipline (background in healthcare or social work preferred). Excellent communication and interpersonal skills. Compassionate attitude and a genuine interest in elder care. Ability to manage field visits independently. Own two-wheeler is mandatory. Basic proficiency in MS Office and digital tools.
