Job Description

Key Responsibilities

  • Conduct regular visits to members' homes to assess their wellbeing and requirements.
  • Coordinate healthcare and support services as per the members care plan.
  • Liaise with family members, doctors, hospitals, and service partners.
  • Maintain detailed care records and update internal systems regularly.
  • Handle member queries and provide empathetic, solution-oriented support.
  • Assist in onboarding new members and conducting care assessments.

Requirements

  • Graduate in any discipline (background in healthcare or social work preferred).
  • Excellent communication and interpersonal skills.
  • Compassionate attitude and a genuine interest in elder care.
  • Ability to manage field visits independently.
  • Own two-wheeler is mandatory.

  • Basic proficiency in MS Office and digital tools.
