Qualifications Experience in care coordination, patient assessment, and case management Strong knowledge of elder care, home healthcare, and rehabilitation services Excellent communication, interpersonal, and problem-solving skills Leadership and organizational skills to effectively manage care teams An empathetic, patient-centric approach and the ability to build trust with clients and families Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field; additional certifications in healthcare or care management are a plus Proficiency in documentation and knowledge of basic health and safety protocols Prior experience in a similar healthcare role is preferred
