Date ______________________ Prepared By: HR/Personnel Dept. Title: Clinical Assistant Department: Medical Job Title: Clinical Assistant Reports to: Medical Admin Major Duties 1. Clinical Assistants have to follow the following duty timings strictly. Morning: M=8am to 4pm, Evening: E=2pm to 10pm, E1= 3pm to 10pm, Night: N= 9pm to 9am. 2. You are supposed to know all the details of all the patients admitted in your ward. 3. Take rounds of patients thrice in 8 hours duty. 4. Chit-chatting at nursing stations should be avoided. 5. Have a personal talk with patients about his complains. 6. Check B.P. manually i.e., with sphygmomanometer stethoscope. 7. Follow all the orders of consultants check whether all the orders are being followed properly by nurses. 8. Put the consultants order in nursing treatment chart check whether nurses are following it properly. 9. Fill the case sheets in all respects; i.e. name of the patient, time, date, clinical notes, investigations, report charts etc. 10. Attend every consultants in their rounds report them periodically about status of patients also about reports of investigations. 11. You are not supposed to leave the assigned place in your duty timings, Lunch should be taken in the pantry area of your department (Not in groups). Tea/coffee/snacks can be ordered from canteen to your site. Frequent movements to canteen is to be avoided strictly. 12. Any leaves, half days, compensatory off, overtime should be communicated at prior date to Medical admin department in written format. 13. Late in or early out is not allowed; more than 3 late marks will be considered as a deduction of day salary. 14. RMO s should put Notes, date, time, signature and stamp in their daily rounds 15. Signature of the consultant is mandatory on the rounds ensure by Duty RMO. 16. All the related reports and investigations should have a through follow up. 17. Any changes in medication or special orders should be communicated with the RMO s on the floor and should be properly documented in notes / Treatment chart. 18. Discharge cards should be prepared and all the heads should be completed correctly, one day prior without fail to reduce the discharge time. (Patient relative sign on discharge card is mandatory. 19. Leaves or any emergency should be communicated to the respective consultant and medical administration. 20. Any last minute changes in duty should be communicated and Locum should be arranged

21. Death summaries should be prepared within 24 hrs.
22. Any procedure should be completed and should be documented on procedure monitoring form. 23. Nephrology resident should take rounds in the OPD dialysis unit and should document the same. 24. Chemotherapy orders should be confirmed and given in the stipulated time to reduce any inconvenience to the patent. 25. OT notes should be duly signed by the consultant. 26. Unless the file complete in all the aspects it would be responsibility of Resident RMO s to get it complete. 27. You have to wear a clean apron, I-card carry your own stethoscope before entering in hospital premises carry it in your duty hours without fail. 28. You should not leave the assigned place without giving over to the reliever. 29. Laptops, Tablet PCs other such gadgets are not allowed in the hospital premises, Use of mobile phones is restricted for emergency use only. 30. Behavior with colleagues should be genuine; professional etiquettes should be followed while talking with seniors, juniors also with other staff. Due respect should be given to female colleagues. 31. Patient care is the main area of concern for Clinical Assistants, any negligence in patient care is considered as misconduct management will take strict action about it. 32. Protocols of prescriptions: Pasting of patient s sticker on prescription Doctor s stamp with sign is must Drug s names should be in capital letters Nurses are not authorized to write prescriptions Min. drugs to be prescribed to RGJAY patients preferably in generic forms (Unless life threatening conditions) 33. Protocols for clinical notes: Clinical Assistants should write notes for at least 3 times in his/her duty 1st : when he comes for the duty 2nd : after consultant s rounds (Consultant s name must be there) 3rd : before leaving the duties And additionally on new event or procedures etc. (i.e. Event capturing) Notes must include Diagnosis complains of patients 34. Any deviation from the above duties of Clinical Assistants will be punishable. Management reserves the right to amend the same at their discretion from time to time as per the job rotation other such requirement
