JOB PROFILE — HEALTH PROMOTERPosition Title: Health PromoterDepartment: Outreach & Community EngagementReporting To: Outreach Coordinator / Program ManagerWork Type: Full-Time / Part-Time (Field + Clinic)Location: Community Areas, Health Camps, Clinics1. Role Summary

The Health Promoter is responsible for educating communities, improving health awareness, assisting in screening camps, and connecting patients to clinic services. They serve as the link between the foundation and the community to promote preventive healthcare.

2. Key ResponsibilitiesA. Community Outreach

  • Conduct door-to-door visits, group meetings, and awareness drives
  • Identify high-need households (elderly, chronic disease, women, children)
  • Maintain visit records and feedback

B. Health Education

  • Deliver structured health sessions on:
  • Hygiene and sanitation
  • Nutrition
  • Skin & hair care
  • Eye health & dental hygiene
  • Lifestyle diseases (Diabetes, BP)
  • Use pamphlets, flip charts, and digital videos

C. Screening & Camp Support

  • Assist in BP, RBS, BMI screening
  • Help in patient registration at camps
  • Manage crowd flow and queue systems
  • Conduct basic triage (under supervision)

D. Referral Support

  • Refer symptomatic individuals to:
  • General Physician
  • Dermatologist
  • Dentist
  • Eye Clinic (Optometrist/Ophthalmology)
  • Track referrals and follow-up for compliance

E. Behavioral Change Communication (BCC)

  • Identify gaps in local health practices
  • Encourage early treatment seeking
  • Reduce stigma around skin diseases, dental care, and mental health

F. Documentation & Reporting

  • Maintain daily MIS
  • Complete household visit forms
  • Submit weekly and monthly progress reports

3. Skills & Competencies

  • Good communication skills (local language + basic English)
  • Empathy and community-friendly behavior
  • Basic medical knowledge (training provided)
  • Ability to work outdoors for field visits
  • Mobile app data entry capability

4. Qualifications

  • Minimum: 10th / 12th Passed
  • Preferred: Diploma in Health Worker / ANM / Community Development
  • Experience: 0–2 years in outreach, NGOs, or clinics
  • Foundation provides initial + refresher training

SOP — HEALTH PROMOTER1. Purpose

To standardize the daily activities, health promotion tasks, and patient-referral workflow for Health Promoters to ensure consistent, high-quality community engagement.

2. Scope

Applies to all field areas, health camps, and clinic-based outreach activities under the Foundation.

3. Responsibilities

  • Follow scheduled routes
  • Educate communities
  • Identify at-risk patients
  • Maintain accurate records
  • Coordinate with clinic teams and outreach supervisors

4. Daily Workflow SOP4.1 Pre-Field Preparation (9:00–9:30 AM)

  • Collect IEC materials (pamphlets, charts)
  • Check mobile/tablet battery
  • Review daily visit plan
  • Update yesterday’s pending entries

4.2 Community Field Visits (9:30 AM – 1:00 PM)

Step-by-step SOP:

  • Visit households as per route map
  • Introduce yourself:
  • “Namaste, I’m from
  • [Foundation Name]
  • . We provide preventive healthcare awareness and screening.”
  • Identify health needs (ask standard questions from checklist)
  • Provide education on relevant issues
  • Record details in digital/physical forms
  • Refer patients who require consultation
  • Invite community members to upcoming health camps4.3 Mid-Day Reporting (1:00–1:30 PM)
  • Update MIS entries
  • Send quick summary to Outreach Coordinator

4.4 Afternoon Group Activities (2:00–4:00 PM)

  • Conduct group sessions in schools, mohalla groups, women’s groups
  • Use flip charts, posters
  • Conduct BP/RBS screening if authorized

4.5 End-of-Day Documentation (4:00–5:00 PM)

  • Complete daily log
  • Update referral follow-up list
  • Submit final report

5. Referral SOPWhen to refer:

  • Eye issues: Blurred vision, headaches, eye strain
  • Dental: Pain, bleeding gums, suspected caries
  • Skin/Dermatology: Persistent rashes, acne, infections
  • General Physician: Fever, chronic cough, diabetes/BP signs
