  Conduct psychosocial assessments of patients and their families.

  Develop and implement individualized care plans in collaboration with psychiatrists, psychologists, and other healthcare professionals.

  Provide counseling, psychoeducation, and support to patients and caregivers.

• 
  Facilitate access to community resources, social services, and rehabilitation programs.

• 
  Conduct home visits and community outreach for follow-up and support.

• 
  Advocate for patients’ rights and mental health awareness in the community.

• 
  Maintain accurate records of patient interactions and progress.

• 
  Organize and participate in mental health awareness campaigns, support groups, and workshops.

• 
  Collaborate with governmental and non-governmental organizations for patient support and rehabilitation.
