Job Description

Key responsibilities of a patient coordinator include: Patient Scheduling and Management: Scheduling appointments, follow-ups, and referrals, ensuring smooth patient flow and continuity of care. Care Coordination: Developing and implementing care plans, coordinating with healthcare providers to ensure all aspects of the plan are executed effectively, and monitoring patient progress. Communication and Information Management: Acting as a point of contact for patients, families, and healthcare providers, facilitating clear communication, and providing updates on patient status. Patient Education and Support: Educating patients about their treatment options, insurance benefits, and financial assistance programs. Record Keeping: Maintaining accurate and up-to-date patient records, including care plans, appointment schedules, and communication notes. Problem Solving and Advocacy: Identifying and resolving any issues that may arise during the care process, advocating for patients' needs, and ensuring they receive appropriate care.
