Role Overview:
We are seeking an experienced Call Center Specialist with a strong background in US healthcare processes and chronic care management (CCM) services. The ideal candidate will be fluent in English, empathetic, detail-oriented, and able to provide professional telephonic support to patients while ensuring compliance with US healthcare standards.

Key Responsibilities:

  • Conduct patient outreach calls for Chronic Care Management (CCM) services.
  • Maintain accurate documentation and update patient health records in the EMR system.
  • Educate patients on their care plans, medications, and follow-up schedules.
  • Coordinate care between patients, physicians, and healthcare staff.
  • Ensure compliance with HIPAA and US healthcare regulations.
  • Handle patient queries with professionalism and empathy.
  • Meet call volume, quality, and performance metrics.

Qualifications & Requirements:

  • Minimum 3 years of experience in a healthcare call center role (US healthcare experience required).
  • Proven experience in Chronic Care Management (CCM) or related care coordination services.
  • Fluent in English (both spoken and written) with excellent communication skills.
  • Strong organizational and documentation skills.
  • Familiarity with EMR/EHR systems (experience with eClinicalWorks preferred, but not mandatory).
  • Ability to work independently in a remote setting.
  • Detail-oriented with strong problem-solving skills.
