Diabetes Educator

We are looking for field Counsellors willing to work as Diabetic Educators with the following qualifications & experience : Qualification Requirements- Graduation in Science/paramedical background preferred – Nursing, MSW, nutritionist, BPTh Counsellor. For candidates from non-Science background past experience/ special educational qualification in counselling is mandatory Past experience in counselling preferred Should be fluent in English ,Hindi and regional languages Brief Profile PSP Counsellor (ALP) Role of a counsellor involves meeting patients, in a day meeting at least 7 to 8 patients at their residences. Meeting patient within 24 hours of receiving request to contact Sharing the essence of the Patient Support Program Sharing essential information about the disease, broad treatment guidelines and lifestyle modifications Carefully listening to the concerns and difficulties faced by the patient Emphasizing the importance of compliance to therapy suggested by the treating physician Timely follow-up with patients to assess patient progress towards therapeutic outcomes and also to ensure that the rapport and timely communication to the respective doctor. Analysis and documenting of reasons for drop-out. Taking doctor and patient consent, and proper maintenance of consent forms. Supporting queries from patients not enrolled in the program. Value Addition to Doctors Customizing the patient compliance program to the needs of individual doctors and ensuring timely and high-quality patient servicing, care and counseling. Sharing patient progress with doctors.
