Role Description This is a full-time on-site role for a Case Manager located in Kolkata. The Case Manager will be responsible for coordinating and managing patient care, providing support and advocacy for patients, and ensuring that patients receive the best possible treatment. Day-to-day tasks include developing personalized care plans, monitoring patient progress, and collaborating with healthcare providers to ensure optimal patient outcomes. Qualifications Experience in patient care coordination and case management Skills in developing personalized care plans and monitoring patient progress Strong communication and interpersonal skills Ability to collaborate effectively with healthcare providers Excellent organizational and time-management skills Proficiency with electronic health records (EHR) systems Bachelor's degree in Nursing, Social Work, or a related field Experience in the healthcare industry is a plus Show more Show less
