Role Overview The Case Manager will provide holistic, long-term support to vulnerable families in the community by: Conducting assessments and developing case plans Coordinating with government and NGO services Monitoring progress through home visits and reports Supporting families to become self-reliant through structured graduation plans Ensuring documentation and compliance for donors and internal systems Key Responsibilities Conduct family assessments and develop support plans Coordinate access to services and referrals Monitor progress and outcomes through regular follow-ups Facilitate sustainable community-led solutions and exit strategies Maintain thorough case documentation and donor reporting Who Can Apply? About Udayan Care Udayan Care is a nationally recognized NGO working for over 24 years to uplift disadvantaged children, women, and youth through quality care, education, and livelihood programs. The organization runs an Aftercare Outreach Program (AOP) supporting care leavers (18+) through education, skilling, and mentoring, helping them transition into independent adulthood. Role Purpose To lead and coordinate the implementation of the Aftercare Outreach Program (AOP) in Gujarat, ensuring smooth operations, stakeholder engagement, youth mentorship, financial management, and program reporting.
