Case Coordinator

Serve as the primary point of contact for parents, caregivers, and therapists regarding each child’s program.

Conduct initial intake, collect developmental history, and assist in enrolment and assessment processes.

Coordinate schedules for therapy sessions (OT, PT, Speech, Special Education, etc.) and parent training programs.

Maintain and update each child’s Individualized Family Support Plan (IFSP) or Individual Therapy Plan.

Track developmental progress and ensure interventions are delivered as planned.

Facilitate regular review meetings with parents and the multidisciplinary team.

Provide guidance, resources, and emotional support to families.

Document case notes, session attendance, progress reports, and follow-up actions.

Assist in referrals to external medical, developmental, or psychological services when needed.

Ensure confidentiality, safety protocols, and compliance with early intervention guidelines.

Support awareness programs, parent workshops, outreach, and community engagement activities.
