Job Description

Medical Records Technician – Key Responsibilities

  • Patient Record Management
  • Collect, organize, and maintain inpatient and outpatient medical records.
  • Ensure proper filing, retrieval, and safe storage of patient records (both physical and electronic).
  • Assign and manage medical record numbers.
  • Data Entry & Documentation
  • Enter patient demographic, clinical, and diagnostic data into the hospital information system (HIS).
  • Maintain accuracy and completeness of patient charts and records.
  • Ensure timely updating of admissions, discharges, transfers, and deaths.
  • Confidentiality & Compliance
  • Maintain confidentiality of patient health information in line with HIPAA or local regulations.
  • Follow hospital policies regarding record access and release of information.
  • Coding & Indexing
  • Assist in medical coding (ICD-10/ICD-11, CPT) for diagnoses and procedures.
  • Index patient records for statistical and research purposes.
  • Reporting & Statistics
  • Prepare daily, monthly, and annual hospital statistics (admissions, discharges, births, deaths, bed occupancy, etc.).
  • Assist in generating reports for management, government agencies, and accreditation bodies (e.g., NABH, JCI).
  • Quality Assurance
  • Check records for completeness, accuracy, and legibility.
  • Follow-up with consultants/nurses for missing reports or incomplete documentation.
  • Participate in audits and support hospital accreditation requirements.
  • Legal & Insurance Support
  • Provide certified copies of medical records for medico-legal cases and insurance claims after due authorization.
  • Maintain records as per legal retention policy.
  • Coordination & Communication
  • Coordinate with doctors, nurses, billing, and administrative staff for timely documentation.
  • Assist patients/relatives in obtaining discharge summaries, certificates, or records (as per policy).
  • Digital Records & Technology
  • Operate Electronic Medical Records (EMR) systems.
  • Scan, digitize, and archive paper-based records.
  • Ensure backup and data security of hospital records.
  • General Responsibilities
  • Assist MRD Officer/Incharge in day-to-day operations.
  • Participate in departmental meetings, training, and quality improvement initiatives.
  • Ensure records are stored as per hospital retention schedule (short-term and long-term archiving).

Job Type: Full-time

Pay: ₹10,832.36 - ₹29,995.68 per month
