Job Description

Key Responsibilities: Patient Support & Counseling Conduct psychosocial assessments of patients and their families. Provide individual, family, and group counseling sessions. Assist patients in coping with illness, treatment processes, and life adjustments. Care Coordination & Case Management Collaborate with doctors, nurses, and clinical psychologists to develop holistic care plans. Monitor patient progress and document interventions accurately. Advocacy & Resource Facilitation Educate patients and families about their rights, treatment options, and available social services. Community & Outreach Activities Participate in health awareness programs, workshops, and seminars. Documentation & Reporting Maintain case records, reports, and patient files in compliance with hospital policies. Prepare reports for management, audits, and research purposes. Required Qualifications: Master’s degree in Social Work (MSW) from a recognized university. Prior experience in clinical, hospital, or healthcare social work is NOT required Knowledge of mental health, counseling techniques, and psychosocial interventions.
