To review and analyse the Medical Errors to identify the factors responsible for the error to minimize the risk to the patient
To coordinate and participate the Root Cause Analysis for errors and events if required
To conduct the various audit for Medication Error, International Patient Safety Goal, Safety Audits etc
Make summaries and report on Patient Safety Indicators in usable and actionable formats.
Based on identified risks, gives recommendations on processes for conducting proactive risk assessments to the respective committee
Daily contacts with hospital staff to maintain an understanding of the hospitals patient safety practices, and to be an accessible resource to staff in matters of patient safety.
To collect and analyze the Quality and Safety Indicators on monthly basis as per the accreditation requirement
To participate in Internal assessment and self-assessment program to identify the area for the improvement and to ensure the compliance with the accreditation standards
Develop, Implement and maintain compliance to the policies, procedures and quality management system as detailed in the Quality manual and Hospital wide Policy and Procedures.
To ensure that the approved and updated Policy and Procedure are maintained and followed
