Job Description

Men / Women between 21-35 years of age who passed a minimum of 12th Standard (graduates would be preferred), have basic knowledge of reading and writing English and moderate understanding of the use of smartphones. They would only need to work on this activity in the nearby two blocks where they reside and have a target to visit a minimum of 20 households daily—preference for those with motorbikes.   1.Preparation and Planning Review daily targets and plan household visits within the assigned blocks. Ensure the Outreach Application, digital Snellen chart, and other tools are fully functional. 2.Household Visits Visit each household as per the daily plan, introducing themselves and explaining the purpose of the visit. Obtain consent from the household for data collection and eye health screening. 3.Data Collection Capture detailed demographic data, pictures and eye health information using the ‘Salakara’ Application. Conduct a vision test using a digital Snellen chart. Ask structured questions to gather information on eye health symptoms and past eye treatments. 4.Referral and Follow-Up Identify individuals needing further examination based on the screening results and refer them to the base hospital. Provide information on the next steps and how to avail of services at the base hospital. 5.Reporting and Feedback Submit daily/weekly/monthly reports on visits conducted, data captured, and referrals made through the Outreach Application. Gather and report any feedback or observations from the field that could improve outreach activities. 6.Promotions and Facilitations Distribute and install posters/banners/pamphlets  Arrange outreach camps, school and anganwadis screenings, speciality screening camps, vision van deployment, and similar activities. Arrange community meetings with key members. Arrange and facilitate partnerships with other organisations in the catchment area. Data Fields to be Captured 1.Demographic Data Household Address (with GPS coordinates) Name of Each Individual Screened Age and Gender Contact Information (Phone number, if available) 2.Vision Test Results Results of the digital Snellen chart test for each eye 3.Eye Health Symptoms and History Presence of any eye health symptoms (e.g., difficulty seeing at night, frequent headaches, blurred vision) History of eye diseases or conditions Past eye treatments and outcomes Use of eyeglasses or contact lenses 4.Photographic Documentation A picture of each individual was screened (with consent) for record keeping and identification.   Referral Criteria to the Base Hospital 1.Vision Test Results Individuals cannot read 6/12 lines (digital Snellen chart) with either eye. 2.Reported Symptoms Complaints of frequent headaches, blurred vision, or difficulty seeing at a distance or close up. 3.Past Eye Health History Individuals with a history of eye diseases or conditions require follow-up or further examination. 4.Immediate Concerns Any visible signs of eye health issues noted during the screening, such as eye discharge, redness, or apparent eye injuries.
