HEAPS is building a robust "Healthcare Network" and driving a "Value-Based Care" model to transform patient outcomes and system efficiencies. Key Responsibilitie:- Provide patients with psychosocial support to help them manage chronic, acute, or terminal health conditions. Communicate effectively with patients post-discharge to understand their health status and offer necessary counseling. Enroll new patients into the system when they initiate contact. Manage both outbound and inbound calls with patients to follow up on their recovery and health needs. Offer emotional and informational support to patients dealing with a wide range of medical conditions. Required Skills:- Excellent verbal communication with the ability to articulate clearly and confidently. Strong active listening skills and empathy. Patient-centric mindset with a genuine passion for supporting individuals in their health journey. Qualification:- Minimum of a Bachelor's Degree in a Clinical, Medical, or Biology-related field. Preferred qualifications include backgrounds such as: Paramedics Clinical Psychologists Physiotherapists Dietitians Nutritionists Or similar related healthcare fields.
