Position Summary:We are looking for a compassionate and proactive Health Care Coach to join our team. This role involves providing psychosocial support to patients, guiding them through post-discharge recovery, and ensuring continuity of care through effective communication and counseling. Key Responsibilities:Provide psychosocial support to patients dealing with chronic, acute, or terminal conditions. Follow up with patients post-discharge to understand health status and offer counseling support. Enroll new patients into the HEAPS system through inbound calls. Maintain accurate documentation of patient interactions and care status. Serve as a trusted liaison between patients and healthcare providers, enhancing overall care experience. Skills Required:Strong communication skills with clarity and empathy. Active listening skills with the ability to understand patient concerns thoroughly. Passion for patient care and a service-driven mindset. Ability to handle sensitive conversations with professionalism. Qualifications:Minimum Bachelor’s Degree in Clinical / Medico / Biology-related background. Preferred: Paramedics, Clinical Psychologists, Physiotherapists, Dietitians, Nutritionists, or similar fields.
