Job Description

Role & responsibilities

Conduct psychosocial & risk assessments at admission and follow-up.
Document psychosocial care plans aligned with medical teams.
Provide brief, evidence-based interventions, crisis counselling and safety planning.
Coordinate medically safe discharge and community referrals (psychiatry, rehab, NGOs).
Deliver family/caregiver psychoeducation on meds, relapse prevention and post-op care.
Maintain clinical records (SOAP notes, risk assessments, discharge summaries).
Join ward rounds, case conferences and clinical audits; support QI and outcome tracking.
Supervise trainees and complete mandatory clinical trainings.

Preferred candidate profile

Qualification:
MSW (Clinical/Medical & Psychiatric)
Clinical exposure:
At least
one clinical internship / field placement
in psychiatry, hospital social work, community mental health or rehabilitation during course. (Freshers with strong internship logs preferred.)
Language:
Good spoken & written
Tamil + English
