Job Description

The Patient Care Coordinator role involves ensuring the efficient coordination and delivery of patient care services. You will work closely with patients, families, and healthcare providers to schedule appointments, manage patient records, and provide support throughout their healthcare journey. As the main point of contact for patients, families, and healthcare teams, you will schedule appointments, coordinate referrals, and assist in follow-up care. Providing patients with information about treatments, procedures, and tests is a key aspect of the role to ensure they are informed and comfortable with the healthcare process. Managing patient records, updating medical charts, and ensuring accurate documentation compliant with privacy regulations (HIPAA) is part of the administrative support responsibilities. Handling insurance verification, pre-authorizations, billing inquiries, and assisting with insurance claims are also crucial tasks. As a Patient Care Coordinator, you will act as an advocate for patients" needs, address concerns, and work with healthcare providers to resolve issues. Navigating complex healthcare systems, referrals to specialists, and accessing community resources are essential aspects of patient advocacy. Collaboration with doctors, nurses, and other healthcare professionals is necessary to ensure seamless care for patients. Coordinating care plans with multidisciplinary teams and participating in meetings to review patient care protocols are key responsibilities. Providing high-level customer service with a compassionate, professional, and empathetic approach is required. Handling patient inquiries and complaints promptly and professionally is essential for maintaining quality customer service. Ensuring that patient care meets healthcare facility standards and complies with regulatory guidelines is vital. Maintaining confidentiality and patient privacy at all times and assisting in quality improvement initiatives related to patient care are important aspects of the role. This is a full-time position with a day shift schedule. One year of hospital experience is required. The work location is in person.,
