Role Description

This is a full-time, on-site role for a Care Manager located in Ahmedabad. The Care Manager will be responsible for overseeing the post-discharge recovery process, providing case management, and coordinating with healthcare providers and patients. Strong communication skills and the ability to work independently are essential for this role.

Qualifications

• Case Management and coordinating with healthcare providers
• Experience in Training and providing support to families
• Skills & Experience in Patient counseling
• Knowledge of Social Services and working with social services professionals
• Background in Medicine or healthcare-related field
• Excellent communication and interpersonal skills
• Ability to work independently and manage multiple cases simultaneously
