The Care Manager will be responsible for overseeing the post-discharge recovery process, providing case management, and coordinating with healthcare providers and patients. Strong communication skills and the ability to work independently are essential for this role. Qualifications Case Management and coordinating with healthcare providers Experience in Training and providing support to families Skills & Experience in Patient counseling Knowledge of Social Services and working with social services professionals Background in Medicine or healthcare-related field Excellent communication and interpersonal skills Ability to work independently and manage multiple cases simultaneously
