Patient Welfare Executive

As a Patients Care Executive, you will be responsible for coordinating with treating consultants, resident doctors, and other medical paramedical and support staff to develop individualized patient care plans from admission to discharge. Your main objective will be to ensure the smooth execution of these plans to facilitate the provision of quality care. Your key responsibilities will include: - Assessing and responding to the needs of patients and their families by coordinating efforts among team members. - Identifying and resolving barriers that may hinder effective patient care. - Collaborating and consulting with physicians regarding patients" progress and discharge planning needs. - Coordinating the discharge planning process to involve all members of the healthcare team. - Maintaining knowledge of available resources and facilities for patients and their families. - Collaborating with personnel at other facilities to ensure smooth and effective patient transfers and transitions. Your qualification should ideally include: - Bachelor's degree in Nursing or relevant field. - Proven experience in a similar role within the healthcare industry. - Strong communication and interpersonal skills. - Ability to work effectively in a team environment.
