Job Summary:

Patient Care Manager

Key Responsibilities:

Care Coordination & Oversight

  • Supervise and coordinate all aspects of patient care services, ensuring that care plans meet medical standards and patient needs.
  • Collaborate with physicians, nurses, therapists, and administrative staff to develop individualized care plans.
  • Monitor and evaluate patient care services to ensure the highest standards of quality and safety.

Team Leadership

  • Manage and support a team of nurses, care coordinators, and other healthcare professionals.
  • Schedule staff, delegate responsibilities, and provide ongoing training and mentorship.
  • Conduct performance evaluations and implement staff development plans.

Patient Advocacy & Experience

  • Serve as a key point of contact for patients and families, ensuring their concerns are addressed promptly and compassionately.
  • Promote a patient-centered culture by advocating for the needs and preferences of patients.
  • Respond to patient feedback and implement improvements to enhance satisfaction.

Compliance & Documentation

  • Ensure adherence to healthcare regulations, accreditation standards, and internal policies (e.g., HIPAA, JCAHO, CMS guidelines).
  • Maintain accurate records, care plans, and documentation in electronic health records (EHR) systems.
  • Participate in audits and quality assurance initiatives.

Process Improvement

  • Analyze workflows, identify gaps in care delivery, and implement best practices to improve operational efficiency.
  • Monitor key performance indicators (KPIs), such as patient satisfaction, readmission rates, and staff utilization.
  • Lead or participate in quality improvement projects.

Preferred Qualifications:

  • Experience in [hospital, home health, long-term care, or specialty clinic] setting
  • Certification in case management (e.g., CCM or ACM) or healthcare quality (e.g., CPHQ)
  • BLS/ACLS certification
  • Familiarity with value-based care models and patient engagement strategies.
