The therapist would develop individualized treatment plans for patients. The therapist conducts and coordinates mental health counseling to include but is not limited to: family, marriage, drug/alcohol referrals and social assessments. Job Responsibilities Completes diagnostic summaries and communicates assessment findings and recommendations to the treatment team. Determines if person needs crisis intervention or long-term care. Counsels patients and families regarding emotional, social and financial consequences of illness and/or disability Provides mental health counseling to include crisis intervention and short-term counseling for patients and families. Enters treatment, writes patient progress notes, updates, continuing care plans, necessary consents, referrals and follow-up activities in the patient's chart Participates in continuing education programs for professional growth. Develops professional referral relationships and resources to help patients in ways not available through the health center system itself.
