An exciting opportunity has arisen within the NHS Greater Glasgow and Clyde Learning Disability Health Check Team for a Band 5 Staff Nurse to support the implementation of Learning Disability Health Checks.
The NHS Greater Glasgow and Clyde Learning Disability Health Check Team is a Nurse-Led service which implements the Scottish Government Directive “The Annual Health Checks For People With Learning Disabilities (Scotland) Directions 2022”.
You will be responsible for the undertaking of health checks within the board area both within clinic and community environments as well as in the individual’s own home. As part of the health check you will determine appropriate recommendations and referrals based on your assessment and the most up-to-date evidence-based practice. You will work with the wider primary care and learning disability services to reduce barriers to accessing health services for this population group.
You should be experienced in working with individuals with learning disabilities and have excellent knowledge of health needs of people with learning disabilities.
Shift Pattern - Monday-Thursday 08.30-4.30 , Friday 08.30-4
