As a Medical Officer at Sukino Continuum Care Facility (CCF) in Hebbal (Manyata) on Night Shift, your role involves active participation in the clients" admission process from a medical, holistic, and continuum care point of view. You will need to understand the needs of the client and the scope of services needed. The admission process starts with a facility visit or connect where the client's clinical details are provided to you. If the provided information is inadequate, you are required to discuss with the primary or referring clinician/team. Your responsibilities include conducting a pre-CCF admission assessment, recommending CCF admission if the client meets the criteria, and providing appropriate care measures. You will also be responsible for creating a detailed initial assessment and holistic continuum plan of care within specific timeframes of the client's admission to CCF. On a daily basis, you will monitor and chart a care plan for clients at the Continuum Care Facility, ensuring their safety and adhering to regulatory standards. You will also conduct regular family meetings to update stakeholders on the continuum care plan and address any queries related to the clinical care plan. Additionally, you will liaise with the medical community for holistic client care, plan the discharge process in coordination with the therapy department, and actively participate in maintaining and improving the quality of healthcare at the facility. Attendance at regular team meetings and professional clinical meetings is mandatory to synchronize care thought processes and implement safe clinical guidelines.
