Clinical Pharmacist

Key Responsibilities

• Medication Therapy Management
• Review and analyze patients’ medication regimens for appropriateness, safety, efficacy, adherence, and cost-effectiveness
• Direct Patient Care
• Counsel patients on medication use, side effects, lifestyle integration, and health behavior
• Conduct medication reconciliation upon admission, during hospitalization, and at discharge
• Interdisciplinary Collaboration
• Join medical rounds and discuss therapeutic plans with physicians and nursing staff
• Provide consultations to healthcare providers on dosing, toxicity, drug interactions, and evidence-based alternatives
• Safety and Quality Assurance
• Monitor for adverse drug reactions and medication errors; implement prevention strategies and quality
• Support formulation of medication policies and clinical guidelines .
• Education & Mentorship
• Train and guide pharmacy interns, technicians, and residents
• Lead patient and peer educational sessions, continuing education
• Research & Protocol Development
• Participate in clinical research, audit projects, and develop evidence-based protocols
• Administrative Support
• Maintain accurate documentation, manage pharmacy systems, support formulary reviews and inventory control .

Qualifications Required : Pharm D

Experience Required: 1 to 3 year

Skills Required :

1. Present personality with good communication skills.

2. Should be good team player.

3. Tech savvy and keen to learn new things

Job Types: Full-time, Permanent

Pay: ₹25,000.00 - ₹30,335.83 per month
