Access Case Manager

As an Access Case Manager, your main role is to coordinate person-centered services for vulnerable individuals, helping them navigate complex systems to access necessary treatment. Your key responsibilities include: - Conducting brief biopsychosocial assessments to determine fit in programming and level of care. - Building rapport with referred clients and community stakeholders for an optimal customer experience. - Assisting prospective clients in accessing necessary services. - Verifying funding for client admissions and determining financial responsibility. - Planning and arranging service initiation for prospective clients. - Maintaining ongoing communication with clients regarding referrals, admissions, and financial responsibilities. - Monitoring system waitlists and completing documentation to clarify client needs. - Supporting smooth transitions to care teams and maintaining relationships with referral partners. - Performing data entry, maintaining accurate documentation, and keeping track of the daily census log. Qualifications Required: - Bachelor's degree in Human Service or Business related field. - Valid license in social work or counseling domain (LSW, LGSW, LISW, LICSW, LADC) OR at least 4 years of case management experience. - Prior experience or knowledge of substance use and co-occurring disorders with at least 1 year in a co-occurring program. - Three or more years experience in a chemical dependency facility (preferred). - Two or more years of sales, marketing, or business development experience (preferred). - CPR certification (or ability to obtain before hire). - Knowledge of all rules, regulations, and requirements of the assigned program. - Computer literate with proficiency in documentation and communication tools. - Flexibility to manage varied responsibilities in a fast-paced environment. - Ability to work independently with minimal supervision. - Possession of a valid MN driver's license.,
