Job Description

The Social Worker at Prayatna plays a pivotal role in providing holistic support to children and families by coordinating care, conducting assessments, and ensuring a seamless service delivery experience. You will work collaboratively with a multidisciplinary team to address the psychosocial needs of clients and their families. You will conduct comprehensive intake assessments to identify clients" developmental challenges, psychosocial needs, and overall well-being. Develop client profiles to guide therapy interventions and support services. Assign clients to appropriate therapy departments and collaborate with multidisciplinary teams to design and implement individualized care plans. You will also monitor progress, modify interventions in consultation with therapists and families, and provide counselling to parents and caregivers to help them understand therapy processes and support their child's development. As a Social Worker, you will advocate for disability certification, facilitate access to necessary services and entitlements, and develop and implement discharge plans to ensure families have the resources and knowledge required for continued progress. Moreover, you will organize awareness campaigns, build networks with local medical professionals, schools, and community groups, and maintain detailed records of client interactions, progress, and assessments. To qualify for this role, you should have a Master's degree in Social Work (MSW) from a recognized university and a minimum of 1-2 years of experience in a healthcare, rehabilitation, or child development setting. Strong interpersonal and communication skills, proficiency in psychosocial assessments and counselling, ability to work collaboratively in a multidisciplinary team environment, and knowledge of developmental challenges and therapeutic interventions are essential skills required for this position. Proficiency in English and Malayalam is also essential. This is a full-time position with benefits including leave encashment, paid sick time, and paid time off. The work schedule is during the day shift, and there is a performance bonus opportunity. Relocation to Ernakulam, Kerala, or reliable commuting before starting work is required. A preferred educational background is a Master's degree, and a minimum of 1 year of social work experience is preferred. Work location is in person.,
