Once the development team hands over the builds to the QA team, QA executes all the scenarios they have documented, reports defects in JIRA and tracks all the issues in JIRA until they are fixed. When all the user stories/ features are tested we release the feature to our production server and keep a back track on the production server while it's being used by our clients.
