Role Description

This is a full-time, on-site position based in Kaushambi for the role of Home Care Coordinator. The Home Care Coordinator will be responsible for overseeing home care services, ensuring proper case management, maintaining effective communication with patients and their families, and providing excellent customer service. The role includes coordinating care plans, scheduling and managing services, and addressing patient concerns to ensure that high-quality home care is delivered seamlessly.

Qualifications

• Skills in Care Coordination and Home Care services to manage and oversee patient care requirements effectively.
• Proficiency in Case Management to develop care plans and track patient outcomes.
• Strong Communication skills to interact with patients, families, and healthcare professionals with clarity and empathy.
• Customer Service expertise to ensure a positive experience and address clients' concerns.
• Ability to work collaboratively in a fast-paced, healthcare-centric environment.
• Relevant educational background or certifications in healthcare, nursing, or related fields are an added advantage.
