Home Care Coordinator

• Qualification: GNM/B.Sc Nursing/MSW/MHA
• Experience: Minimum 2 years
• Looking for an experienced candidate as a home care coordinator
• Ensure all stakeholders are updated regarding patient condition, services required, and schedules.
• Serve as a key point of contact between the hospital, patient families and home care staff.
• Ensure the timely availability of required equipment, consumables, and medications.
• Maintain accurate records of patient care plans, service delivery, and follow-up.
• Monitor quality of care provided and ensure patient satisfaction.
• Conduct home visit or follow-up calls to ensure proper care delivery.
• Provide orientation and ongoing training to home care staff.
• Educate patients and caregivers on managing health conditions at home.
• Collect patient feedback and assist in quality improvement initiatives.
• Ensure follow-up communication post-visit for continuity of care.
• Track availability of staff and assign duties in coordination with the homecare lead.

Job Type: Full-time

Pay: From ₹20,000.00 per month
