Job Description

1. Preoperative Responsibilities

  • Patient Evaluation
  • Conduct thorough pre-anesthetic check-ups.
  • Assess cardiac function (e.g., via ECG, echocardiography).
  • Identify and optimize comorbid conditions (e.g., hypertension, diabetes, pulmonary disease).
  • Planning Anesthesia
  • Design individualized anesthesia plans based on surgical type and patient condition.
  • Explain anesthesia procedures and risks to patients and families; obtain informed consent.

2. Intraoperative Responsibilities

  • Anesthesia Administration
  • Induce and maintain general anesthesia for complex cardiothoracic procedures (e.g., CABG, valve replacement, aortic aneurysm repair).
  • Use advanced airway and vascular access techniques (central lines, arterial lines).
  • Intraoperative Monitoring
  • Continuously monitor vital parameters: ECG, arterial pressure, CVP, SpO₂, temperature, urine output.
  • Use Transesophageal Echocardiography (TEE) to guide intraoperative decisions.
  • Manage anticoagulation during procedures using CPB (cardiopulmonary bypass) and heparin-protamine protocols.
  • Hemodynamic Management
  • Titrate vasoactive medications and fluids to ensure cardiac stability.
  • Manage cardiopulmonary bypass weaning and respond to crises (arrhythmias, cardiac tamponade, bleeding).

3. Postoperative Responsibilities

  • ICU Management
  • Oversee immediate postoperative care in the cardiac ICU.
  • Assist in ventilator management, sedation, pain control, and inotropic support.
  • Identify and manage complications (e.g., low cardiac output syndrome, bleeding, renal dysfunction).
  • Extubation & Recovery
  • Evaluate readiness for extubation.
  • Ensure smooth transition from mechanical ventilation to spontaneous breathing.

4. Academic, Administrative & Team-Based Roles

  • Training & Teaching
  • Train junior anaesthesiologists, residents, and ICU staff in cardiac anesthesia techniques.
  • Documentation
  • Maintain detailed anesthesia records and contribute to surgical audit meetings.
  • Multidisciplinary Collaboration
  • Coordinate closely with surgeons, perfusionists, intensivists, and nurses to optimize patient outcomes.
  • Research & Development
  • Participate in clinical research, QI programs, and stay updated with advancements in cardiac anesthesia.

Specialized Skills Required

  • Expertise in:
  • Transesophageal Echocardiography (TEE)
  • Cardiopulmonary Bypass (CPB) physiology
  • Advanced hemodynamic monitoring
  • Mechanical circulatory support (IABP, ECMO)
