Job Description

The Discharge Summary Typist is responsible for preparing accurate and timely discharge summaries for patients based on documentation provided by doctors and clinical staff. The role requires strong typing skills, attention to detail, and basic knowledge of medical terminology to ensure that all discharge documentation is complete, standardized, and error-free.

Key Responsibilities:

  • Type discharge summaries from handwritten notes, dictated recordings, or electronic formats provided by treating consultants.
  • Ensure all patient details (name, hospital ID, diagnosis, treatment, dates of admission/discharge) are entered correctly.
  • Maintain consistency in formatting and use of hospital-approved templates.
  • Proofread and correct spelling, grammar, and medical terminology errors before finalizing.
  • Coordinate with doctors and nursing staff to clarify incomplete or unclear information.
  • Submit typed summaries for review and approval by the responsible consultant.
  • Upload final discharge summaries into the hospital’s EMR/HIS system or print as required.
  • Maintain confidentiality of all patient information and comply with hospital data security protocols.
  • Assist in managing discharge summary records, both digital and physical, as per hospital retention policy.
  • Work closely with the medical records and nursing teams to support efficient patient discharge workflows.

Qualifications:

  • Minimum 12th Pass (Higher Secondary); a graduate is preferred
  • Typing speed: Minimum 40–50 words per minute with high accuracy
  • Prior experience in hospital medical records or clinical documentation preferred

Skills Required:

  • Proficient in English typing and medical terminology
  • Good knowledge of Microsoft Word and hospital information systems (HIS/EMR)
  • Strong attention to detail and commitment to quality
  • Ability to handle multiple discharge summaries under time constraints
  • Good communication and coordination skills
  • Familiarity with healthcare documentation standards and confidentiality practices

Desirable:

  • Experience in a NABH/JCI-accredited hospital
  • Understanding of discharge process workflows in healthcare settings
  • Willingness to work in rotational shifts, weekends, or extended hours as needed
