Transplant Coordinator

Patient evaluation and education:

• Educates patients and families about the transplant process, risks, and necessary lifestyle changes.
• Evaluates a patient's readiness for transplant, including their understanding of the process and their need for social and emotional support.

Logistical and medical management:

• Coordinates all necessary tests, evaluations, and appointments with different specialists.
• Acts as the primary point of contact for patients, addressing questions related to insurance, finances, and transportation.
• Manages the transplant list and contacts patients when a donor organ becomes available.
• Monitors patient compliance with medications and other follow-up instructions after the transplant.

Patient and family advocacy:

• Advocates for the patient's needs and ensures they have adequate support systems.
• Collaborates with other members of the transplant team, including doctors, social workers, and dietitians.

Living donor support:

• Evaluates living donors for suitability and manages the entire donation process for them.
• Educates donors on the risks, benefits, and recovery process.

Post-transplant care

• Long-term follow-up:

Coordinators continue to work with patients for years after a transplant, helping them manage long-term medications, monitor kidney function, and deal with any potential complications.

• Ongoing support:

They help patients understand the importance of long-term care and provide support to ensure they can maintain a healthy lifestyle after the surgery

• Location: Amritsar
• Experience: 2 to 5years
• Qualification : Diploma or Professional Certification in Transplant Coordination
