Job Description

Manages and facilitates the complex process of organ and tissue transplantation for patients, serving as a liaison between patients, families, and the medical team to ensure a seamless and supportive care journey from evaluation through post-transplant follow-up
Patient evaluation, education, coordinating testing, managing documentation, counseling patients and families, collaborating with various healthcare professionals, and providing essential emotional support throughout the entire transplant process.
Conducting patient and family education on the transplant process and post-transplant care.
Coordinating comprehensive medical evaluations to determine transplant eligibility
Providing ongoing support and counseling to patients and families, addressing financial, personal, and logistical concerns
Acting as the primary point of contact and liaison for patients, families, and the multidisciplinary transplant team.
Facilitating communication and collaboration among various healthcare providers
Managing patient caseloads and coordinating care across different departments to ensure continuity
Managing all relevant medical records and documentation related to transplantation
Ensuring compliance with legal criteria and ethical guidelines governing organ donation and transplantation.
For donor coordinators, this involves identifying potential organ donors, managing the organ donation process, and coordinating organ recovery and transport.
Ensuring the stability of the donor's organs and overseeing the completion of diagnostic tests for organ placement.
