Job Description

Role & responsibilities

  • Maintain and update patient medical records (both physical and digital).
  • Classify, index, and file records accurately for easy retrieval.
  • Ensure confidentiality and security of patient health information (PHI).
  • Track and manage the movement of patient files within departments.
  • Handle requests for patient records from authorized personnel, insurance, and legal entities.
  • Prepare discharge summaries, death summaries, and maintain admission-discharge registers.
  • Coordinate with doctors, nurses, and other departments for record completeness.
  • Assist in digitization of old records and data entry.
  • Support in medico-legal cases (MLC) documentation and retrieval.
  • Participate in audits, inspections, and accreditation processes (e.g., NABH).
  • Archive and dispose of records as per hospital policy and legal requirements.
