Diabetes Educator

• Provide self-management training and education to type 1, type 2, and gestational diabetic patients. Education includes but is not limited to, signs/symptoms, and respective treatments for hyper glycaemia and hypoglycemia.
• Provide nutritional counselling for diabetes management.
• Converting free trial patients into paid patients and open to new roles.
• Deliver patient education as needed or as directed by the Provider.
• Perform assessments and data collections, establishes outcome criteria; obtains, reviews, and analyses information in collaboration with patients, family members, and health care team members.
• Attend outreach activities such as health fairs for diabetes screening and health education.
• Monitor and evaluate short and long-term patient responses to therapeutic interventions in collaboration with providers; provides and maintains necessary follow-up for patients and families.
• Educate and follow patients with diabetes for the control of blood sugar, lipids, renal function, and prevention of diabetes complications.
• Instruct patients and family members on how to administer insulin, self-monitor blood glucose, an medication education.
• Should have a degree in B.Sc. (Home Science/Dietetics/Food & Nutrition) and Masters in Dietetics/Food & Nutrition,
• Current understanding of industry requirements, processes, and procedures for Dieticians and Diabetes Educators.
• Language Proficiency -

  Candidate must be proficient in English, Hindi and Marathi

• Experience in patient education and counselling.
