Role & responsibilities

Medication Management

• Ensure accurate dispensing of medications, especially for intravenous and chemotherapy preparations.
• Monitor drug therapy regimens for interactions, contraindications, and appropriateness.
• Maintain proper storage conditions and inventory control of all medications.

Compliance & Safety

• Ensure adherence to hospital policies, procedures, and legal regulations.
• Conduct regular audits and inspections of drug administration records and medication areas.
• Promote safe drug disposal practices and pharmacovigilance.

Coordination with Healthcare Teams

• Collaborate with doctors, nurses, and technicians to ensure proper medication administration.
• Provide drug-related information, including side effects, dosage, and interactions.
• Assist in selecting appropriate drug regimens and scheduling administration times.

Patient Care & Education

• Take patient medication histories and counsel them on drug usage and side effects.
• Monitor patient responses to medications and adjust therapy as needed.
• Participate in discharge planning by reviewing medication charts and summaries.

Supervision & Training

• Oversee pharmacy technicians and interns.
• Conduct in-service training for pharmacy staff and nursing personnel.
• Ensure proper compounding techniques and documentation practices.

Record Keeping & Documentation

• Maintain accurate records of drug supply, billing, and investigational drugs.
• Ensure prescriptions are correctly entered into the unit dose system.
• Periodically review and update patient medication charts.

Emergency Response

• Participate in cardiopulmonary and other medical emergencies by providing rapid pharmaceutical support.
• Ensure availability of emergency medications and supplies.
