Job Description

Job Summary

The Social Worker supports individuals undergoing de-addiction treatment by providing psychosocial care, case management, and rehabilitation support. Working closely with the clinical and multidisciplinary team, the Social Worker assists patients and their families in addressing social, emotional, and behavioural challenges, promoting recovery, relapse prevention, and successful reintegration into society.

Key Responsibilities

Patient & Family Support

  • Conduct psychosocial assessments and assist in developing individualized care and rehabilitation plans.
  • Provide supportive counseling to patients and families under supervision.
  • Support patients in coping with withdrawal, emotional distress, and behavioural challenges.
  • Maintain a safe, respectful, and non-judgmental environment that supports recovery.

Program & Rehabilitation Support

  • Assist in planning and conducting group sessions, awareness programs, and rehabilitation activities.
  • Encourage patient participation in therapy, life-skills training, and structured daily programs.
  • Coordinate with families, community resources, and referral agencies for follow-up care and reintegration.

Case Management & Documentation

  • Maintain accurate case records, progress notes, and reports in accordance with centre policies.
  • Support admission, discharge, and aftercare planning processes.
  • Participate in team meetings and contribute to treatment planning discussions.

Personal Qualities

  • Empathetic, patient, and non-judgmental approach to individuals with substance use disorders.
  • Emotional maturity and ability to handle challenging situations professionally.
  • Strong ethical values, including respect for confidentiality and professional boundaries.

Required Skills & Qualifications

  • Master’s degree in Social Work (MSW/MSc. Psychology).
  • Training or internship experience in de-addiction, mental health, or psychiatric settings preferred.
  • Good communication, counseling, and documentation skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Willingness to work flexible hours or shifts as required.
