Clinical Care Manager

We are hiring clinic care manager.

Male and female both are eligible.

1. Care Coordination and Management:  Develop individualized care plans based on patient assessments, goals, and preferences.

 Coordinate with healthcare providers, families, and community resources to ensure the delivery of appropriate services.

 Monitor patient progress and adjust care plans as necessary.

 Oversee patient admissions, transfers, and discharges, ensuring a smooth transition in care.

2. Patient Advocacy and Support:  Act as a liaison between patients, families, and healthcare professionals to ensure clear communication and understanding of the care plan.

 Provide guidance and support to patients and their families regarding treatment options, care procedures, and available resources.

 Advocate for the patient’s needs and ensure their rights and dignity are maintained.

3. Quality Assurance and Compliance:  Monitor and evaluate care outcomes, ensuring continuous improvement and patient safety.

 Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with HIPAA or relevant privacy laws.

4. Reporting and Documentation:  Document all care-related interactions and decisions in the patient’s record, including any changes to care plans, patient conditions, and care team communications.

Pay: ₹40,000.00 - ₹55,000.00 per month
