- Provide comprehensive pharmaceutical care to patients in an acute care setting, ensuring safe and effective medication use
- Collaborate with healthcare professionals to review and assess medication orders for appropriateness, accuracy, and potential interactions- Monitor patient drug therapies and provide appropriate interventions to optimize therapeutic outcomes- Educate patients and their families about medications, including usage, side effects, and potential interactions- Participate in interdisciplinary rounds and contribute to clinical decision-making processes- Maintain accurate records of medication administration and patient interactions- Stay updated on the latest pharmaceutical research and advancements to continually improve patient care

Required Qualifications

- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution
- Active pharmacist license in the state of practice- Completion of a residency in acute care or equivalent experience in a hospital setting- Strong clinical knowledge and ability to apply it in fast-paced, high-pressure environments- Excellent communication and interpersonal skills for effective collaboration with healthcare teams- Proficiency in using electronic health records and pharmacy management systems
