Job Description

Responsibilities

Provide comprehensive pharmaceutical care to patients in an acute care setting, ensuring safe and effective medication use.

Collaborate with healthcare professionals to review and assess medication orders for appropriateness, accuracy, and potential interactions.

Monitor patient drug therapies and provide appropriate interventions to optimize therapeutic outcomes.

Educate patients and their families about medications, including usage, side effects, and potential interactions.

Participate in interdisciplinary rounds and contribute to clinical decision-making processes.

Maintain accurate records of medication administration and patient interactions.

Stay updated on the latest pharmaceutical research and advancements to continually improve patient care.

Required Qualifications

Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.

Active pharmacist license in the state of practice.

Completion of a residency in acute care or equivalent experience in a hospital setting.

Strong clinical knowledge and ability to apply it in fast-paced, high-pressure environments.

Excellent communication and interpersonal skills for effective collaboration with healthcare teams.

Proficiency in using electronic health records and pharmacy management systems.Workplace Culture
